Job summary

Your Health Organization in Blythewood, SC is seeking a Registered Nurse with Hospice experience to join our primary care team as a Nurse Case Manager. You will visit patients at home and in facilities, coordinate with providers, and initiate telemedicine visits.

You must have reliable transportation as travel is required daily. The role supports care management plans, reviews records, and collaborates with interdisciplinary teams to deliver compassionate, compliant care across South Carolina

Qualifications

Must be a Registered nurse with current license in good standing.
Minimum 3 years clinical experience preferred.
Experience in community or home-based settings is desirable.
Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
Strong written and verbal communication skills; basic computer proficiency is required.

Responsibilities

Coordinate care across the patient’s health care team, providers, and caregivers.
Enroll patients in Care Management and initiate a Plan of Care.
Review medical records, complete consents, and document Care Management activities.
Facilitate home visits, facility visits, and telemedicine as needed to support care.
Collaborate with hospice teams and other care entities to ensure quality, compliant care.
