Job summary

UnitedHealth Group’s Optum Care at Home team is hiring a Telephonic RN Case Manager to support D-SNP members with Medicare and Medicaid. You will perform telephonic assessments, develop care plans, coordinate transitions, and remove barriers to care in a high-volume, remote setting.

Requirements include active RN license valid in WA and CO, 2+ years of RN experience, Medicaid LTSS experience, and proficiency with electronic health records.

Qualifications

• Active, unrestricted RN license or multistate license valid in Washington and Colorado by start date.
• 2+ years of professional RN experience.
• Experience coordinating care for patients with complex or high-risk needs.
• Experience with Medicaid LTSS or related Medicaid care management program.
• Experience in case management, managed care, home health, transitions of care or similar settings.
• Proficiency documenting in electronic health records or care management platforms.
• Driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive telephonic assessments of members’ medical, behavioral health, functional, psychosocial, caregiver, environmental and socioeconomic needs.
• Develop, implement, and reassess individualized, person-centered care plans reflecting goals, risks, and independence.
• Coordinate care across Medicare, Medicaid, primary and specialty care, behavioral health, home health and community programs.
• Identify gaps in care and barriers to treatment and connect members with resources.
• Facilitate safe transitions between hospitals, SNFs, rehab, home health and residence with timely follow-up.
• Document assessments, care plans, interventions and outcomes in EHRs.
• Maintain RN licensure and pursue ongoing education and credentials.
