Job summary

CVS Health seeks a compassionate Registered Nurse to coordinate in-home care for members with complex needs in New Jersey. The role is primarily field-based with the flexibility to work from home when not in the field.

You will assess medical, behavioral, and social needs, develop personalized care plans, and collaborate with care teams to ensure holistic, high-quality service while supporting transitions and community resource connections.

Qualifications

• Active RN license in New Jersey is required.
• Minimum 3 years of relevant clinical experience.
• Experience with in-home assessments and care coordination.
• Ability to travel reliably within assigned area.

Responsibilities

• Conduct in-home assessments for members in care programs.
• Evaluate medical, behavioral, and social needs to plan care.
• Collaborate with providers, caregivers, and teams for coordinated care.
• Develop individualized care plans to promote safety and independence.
• Document assessments and care activities in the EHR.
