Qualifications

• Active, unrestricted RN license in New Jersey
• Minimum 3 years of clinical experience in hospitals, home health, or ambulatory care
• Residency in Middlesex County, NJ
• Willingness to travel up to 75% with mileage reimbursed per policy
• Successful completion of NJ Choice Certification: 80%+ exam, state training modules, field mentoring

Responsibilities

• Conduct face-to-face home visits using assessment tools for MLTSS and/or D-SNP/FIDE members
• Perform in-person assessments for non-MLTSS members to determine medical needs and referrals
• Coordinate and collaborate with members, representatives, primary care providers, and care teams
• Schedule and participate in interdisciplinary meetings to advocate for safe transitions and appropriate services
• Develop individualized care plans and authorize services within MLTSS/FIDE benefits
• Document assessments, interactions, and care planning in the EHR
• Mentor new staff as proficiency increases
