Job summary

Qualifications

• Active RN license in state of practice.
• 2+ years of healthcare experience with care management.
• Experience with disability, chronic conditions, LTSS, or equivalent.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Develop and implement care plans and waiver service plans with care team and members.
• Monitor care plans and document interventions to evaluate outcomes.
• Coordinate waiver enrollment and disenrollment processes.
• Promote integrated care across LTSS and behavioral health services.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service approval/denial and collaboration.
• Educate and motivate members using motivational interviewing and Molina guidelines.
• Identify barriers and provide care coordination to address psycho/social, financial, and medical issues.
• Identify critical incidents and develop prevention plans.
• May provide consultation to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% may be required.
