Molina Healthcare is seeking a remote RN care manager to support care coordination for high-need members across Wisconsin. This role involves home visits, waiver management, and multidisciplinary collaboration to ensure quality, cost-effective outcomes. Requires an active RN license and relevant LTSS experience.

Travel up to 40% locally within Dodge, Fond du Lac, and Green Lake counties as needed, with strong emphasis on care planning and coordination.

Qualifications

• RN license must be active and unrestricted in state of practice.
• Experience in health care with care management, managed care, LTSS, or equivalent.
• Proficiency with Microsoft Office and applicable software.

Responsabilités

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and the care team.
• Monitors care plan effectiveness, documents interventions and goal achievement, and suggests changes.
• Promotes integration of services across LTSS, behavioral health, and community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing and Molina clinical guideposts.
• Assesses barriers to care and coordinates psycho/social, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• May provide consultation to peers.
• Care manager RNs may handle complex member cases and medication regimens.
• May conduct medication reconciliation.
• 25-40% local travel may be required.
