Job summary

Molina Healthcare in Wisconsin is seeking a Registered Nurse care manager to support care coordination for LTSS. You will work with a multidisciplinary team to develop and implement care plans, assess medical necessity, and facilitate waiver services.

The role includes home visits, travel 25-40%, and collaboration with caregivers and physicians to improve member outcomes. You will apply motivational interviewing and Molina clinical guidelines to educate and motivate members, monitor plan

Qualifications

• At least 2 years in health care with 1 year in care management or LTSS.
• RN licensed and active in state of practice.
• Experience with disability, chronic conditions or behavioral health settings.
• Ability to travel 25-40% locally.

Responsibilities

• Complete member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and care plans.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Participate in ICT meetings for service approvals.
• Educate and motivate members using motivational interviewing.
• Identify barriers and address psycho/social, financial, and medical obstacles.
• Perform medication reconciliation as needed.
• Travel 25-40% locally as required.
