Job summary

Molina Healthcare is seeking a Registered Nurse to support care management and LTSS activities. The role involves coordinating care across disciplines, performing assessments, and developing individualized care plans for high-need members.

Responsibilities include waiver management, team collaboration, and ensuring timely service delivery with documentation. Travel within Wisconsin is required to visit members and support care continuity.

Qualifications

• At least 2 years in health care with 1 year in care management or LTSS.
• RN license active and unrestricted in state of practice.
• Bachelor's degree may be required depending on state.
• Valid driver's license and reliable transportation for travel.
• Ability to work proactively with strong communication.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and home visits as required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with the member and care network.
• Monitors care plan effectiveness and documents progress.
• Promotes integration of LTSS, behavioral health, and community resources.
• Coordinates ICT meetings and medical approvals as needed.
• Assesses barriers to care and supports members to overcome obstacles.
• May perform medication reconciliation for complex cases.
