Job summary

Molina Healthcare is seeking a registered nurse care manager to support coordination of LTSS for high-need members in Wisconsin. You will assess needs, develop care plans, and collaborate with a multidisciplinary team to optimize outcomes and manage costs.

You will conduct home visits as required, facilitate waiver processes, monitor plan effectiveness, and promote integration of behavioral health, long-term services, and community resources. Local travel will be required.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years in health care; 1 year in care management/LTSS.
• Valid driver's license and reliable transportation.
• Able to work independently and handle multiple priorities.
• Strong verbal and written communication skills.
• Experience with diverse populations.
• Proficiency with Microsoft Office.

Responsibilities

• Complete comprehensive member assessments within timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team.
• Monitor care plan effectiveness and document progress toward goals.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for approval/denial of services.
• Travel up to 25-40% locally as required.
