Job summary

Molina Healthcare seeks a skilled RN Care Manager to support LTSS member care coordination remotely with travel for in-person visits across Wisconsin counties. You will assess needs, develop care plans with clinicians, monitor progress, and coordinate services to improve outcomes while controlling costs.

The role requires clinical licensure, strong communication, and ability to work independently in a dynamic, team-based setting with diverse member populations.

Qualifications

• 2+ years experience in health care, with care management/ LTSS exposure.
• RN licensed and active in state of practice; valid driver's license.
• Experience with diverse populations and high-need members.
• Ability to work independently with minimal supervision.

Responsibilities

• Complete member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and care plans.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of behavioral health and LTSS services.
• Identify barriers and arrange resources to support members.
• Lead ICT meetings for approval/denial of services.
• Travel 25-40% for member visits as required.
