Job summary

Molina Healthcare is seeking an RN Care Manager to support LTSS care coordination and collaborate with a multidisciplinary team to deliver integrated member care nationwide within Wisconsin. The role emphasizes comprehensive assessments, care planning, and driving outcomes while ensuring cost-effective service delivery.

The position requires active RN licensure, 2+ years in health care with care management experience, and ability to travel 25–40% within the state.

Qualifications

• At least 2 years of experience in health care, including at least 1 year in care management.
• Active RN license in state of practice.
• Bachelor’s degree in a health care related field may be required in some states.
• Valid driver’s license and reliable transportation for job-related travel.
• Strong verbal and written communication and time-management skills.
• Proficiency with Microsoft Office or equivalent office software.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Develops and implements care plans with members, caregivers, physicians and others to address member needs and goals.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Facilitates waiver enrollment and disenrollment processes.
• Promotes integration of services across LTSS and behavioral health to ensure continuity of care.
• Assesses medical necessity and authorizes appropriate waiver services.
• Coordinates interdisciplinary care team meetings for approvals and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Identifies barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
