Job summary

UnitedHealthcare is seeking a Licensed Field Care Coordinator to coordinate care for members in Davidson, Wilson, and Sumner Counties, TN. This field-based role requires clinical RN expertise and the ability to travel within the territory to ensure access to services and providers.

You will conduct assessments, develop care plans, educate members, and collaborate with the care coordination team to improve outcomes while maintaining regulatory compliance.

Qualifications

• Current Nursing License (RN).
• 2+ years of clinical RN experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Excellent organization, communication and time management skills.
• Reside in or within commutable distance to Monroe County.
• Access to reliable transportation and valid US driver’s license with ability to travel up to 75% within assigned territory.

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid beneficiaries.
• Incorporate evidence-based clinical practices into care coordination activities.
• Provide member-centered, self-management support and education.
• Ensure appropriate utilization and consistent health benefits application.
• Conduct telephonic and face-to-face assessments per policy and timelines.
• Act as member advocate to resolve barriers to care.
• Coordinate referrals to community resources with members and providers.
• Develop and maintain individual care plans based on identified needs.
