Job summary

Boost Home Healthcare is seeking a Field Registered Nurse to administer skilled nursing care to patients in their homes, following physician orders and the plan of care under supervision. The role emphasizes promoting independence and quality of life through personalized home health services.

The ideal candidate will be a licensed RN with home health experience, valid BLS, reliable transportation, and residency in Lorain County.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN licensure in the posting state.
• Minimum of 1 year nursing experience in home health.
• Current BLS certification.
• Access to reliable transportation.
• Ideal candidate resides in Lorain County (Avon, Elyria, North Ridgeville, Lorain).

Responsibilities

• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

The Field Registered Nurse (Field RN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction of the Director of Patient Care Services.
