Boost Home Healthcare - Northeastern Ohio is seeking a Field Registered Nurse to administer skilled nursing care to patients in their homes. You will evaluate needs, initiate and revise plans of care, and document care conferences in a timely manner while following physician orders.

The ideal candidate is a graduate of an accredited nursing program, licensed in Ohio, with at least 1 year in home health, current BLS, reliable transportation, and flexible scheduling.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of one (1) year of nursing experience in a home health setting.
• Must have current BLS Certification.
• Access to reliable transportation.
• Preferably resides in Strongsville, Brook Park, Parma, Seven Hills, or Independence area.
• Flexible work from home options available.

Responsibilities

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

The Field Registered Nurse (Field RN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction of the Director of Patient Care Services.
