Job summary

Boost Home Healthcare - North Shore (MA) is seeking a dedicated Registered Nurse to provide compassionate, one-on-one home health care to patients in their residence. You’ll perform initial evaluations, plan of care, and ongoing nursing assessments to support independence and quality of life.

The role requires an active RN license, current BLS, at least one year of home health experience, reliable transportation, and a flexible schedule.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of one year of nursing experience in home health.
• Must have current BLS Certification.
• Access to reliable transportation.

Responsibilities

• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

• $43-$52/hr
