Job summary

Boost Home Healthcare is seeking a dedicated Registered Nurse to join our home health team in Colorado. You will evaluate patient needs, plan and adjust care, and document progress to support independent living at home.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of one (1) year of nursing experience in a home health setting.
• Must have current BLS Certification.
• Access to reliable transportation.

Responsibilities

• Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

Job Type and Pay:

Full time

• $43-$52/hr
