Job summary

AndHealth in Columbus, OH is seeking a Patient & Provider Advocate to own patient identification, enrollment, and access coordination within partner practices and EHR systems.

The role requires 5+ years in reimbursement or patient access, a Bachelor's degree, willingness to travel, and the ability to work independently while collaborating with clinicians and staff to ensure white-glove service and timely program enrollment.

Qualifications

• Bachelor’s Degree required.
• 5+ years in field reimbursement, patient access, or clinical operations.
• Willingness to travel regularly; on-site at partner practices most days.
• Familiarity and willingness to use AI tools.

Responsibilities

• Identify eligible patients and enroll them in the program.
• Work within partner practices and EHR systems to identify CDM-eligible patients.
• Queue identified patients for provider review and referral—reduce practice staff burden.
• Conduct outbound outreach to support enrollment and transition.
• Track enrollment funnel and surface process gaps with solutions.
• Serve as on-site expert for medication access challenges.
• Build relationships with practice coordinators and office staff.
• Provide white-glove referrals and enhanced status communication.
• Train staff on referral workflows and reminders.
• Document bespoke workflows with Account Management for continuity.
