Job summary

AndHealth is seeking a Patient & Provider Advocate in Dayton, OH. This full-time role embeds in partner practices, often within their EHR, to identify eligible patients and ensure successful enrollment in programs.

You will queue patients for provider review, conduct outbound outreach, and track enrollment activity to address access barriers. Travel to partner sites is expected regularly.

Qualifications

• Bachelor's Degree required.
• 5+ years in patient access, reimbursement, or clinical operations.
• Comfortable working independently in provider offices.

Responsibilities

• Manage patient enrollment from referral to onboarding.
• Identify CDM-eligible patients in target specialties within partner practices.
• Queue patients for provider review and assist with enrollment transitions.
• Provide outbound patient contact to support enrollment and ensure smooth handoffs.
• Track enrollment funnel and surface process gaps with actionable solutions.
• Serve as on-site expert for medication access challenges.
