Job summary

Qualifications

• High School Diploma or GED-equivalent; lived experience encouraged.
• At least 1 year of related experience in mental health, homelessness services, or case management.

Responsibilities

• Initiates and maintains relationships with clinics and community partners to promote access to services.
• Coordinates client enrollment, eligibility screening, and service linkage.
• Collects data and maintains documentation to meet grant reporting guidelines (PATH, ESG).
• Provides direct client support, case management, and crisis intervention as needed.
• Maintains HMIS and Health Record System data for progress tracking.
