Job summary

Paycom - ATS is seeking a dedicated case manager to support clients in a short-term life management program. You will interview applicants, set goals, and monitor progress while maintaining accurate records in HMIS.

The role includes advocacy for services and coordinating referrals with community agencies. The ideal candidate holds two years of college in Social Work and has two years of experience in social/public service.

Qualifications

• Eligibility interviews and needs assessment conducted per guidelines.
• Establish program goals and monitor client progress.
• Prepare and maintain accurate case records and logs.
• Refer clients to appropriate agencies when needed.
• Enter information into HMIS with accuracy and timeliness.

Responsibilities

• Interview applicants to determine eligibility for the program enrollment.
• Develop needs assessment and establish program goals for eligible clients.
• Prepare and maintain case records and HMIS documentation.
• Mentor clients and model proper hygiene, budgeting, and scheduling.
• Coordinate referrals to external agencies as required.
