Paycom - ATS is seeking a dedicated case manager to provide comprehensive, long-term case management for clients in an established life management program. You will interview, assess needs, set goals, counsel, and advocate to obtain essential services, ensuring compliance with funding requirements.

Qualifications

• Bachelor’s degree in social Work, Behavioral Science, or a related field.
• Three years of progressively responsible experience in direct case management.
• Experience accessing clients’ needs and developing long-term recovery plans.
• Equivalent combination of training and experience accepted.

Responsibilities

• Interviews applicants to determine eligibility for program enrollment based on facility guidelines.
• Establishes long-term goals (three months or more) for eligible clients.
• Counsels clients to modify or stop negative behaviors while in the program.
• Outsources clients to additional counseling resources when needed.
• Maintains progress records and meets with clients regularly to evaluate growth.
• Advocates for clients to obtain services and income supports.
• Ensures compliance with funding requirements.
• Plans life management and related classes to build coping skills.
