Job summary

Molina Healthcare in Illinois is seeking a Family Nurse Practitioner to provide screening, preventive and medical care to members in home health, community, and facility settings. You will perform history and physicals, manage chronic conditions, and coordinate care across the EMR and care teams.

The role requires an active FNP license, AANP/ANCC certification, DEA license, and BLS, with travel up to 120 miles daily, and collaboration with physicians under state regulations.

Qualifications

• Active and unrestricted national certification from AANP or ANCC.
• Current state-issued Family Nurse Practitioner license (FNP).
• DEA license to prescribe (state-specific).
• BLS certification.

Responsibilities

• Provide general medical care and care coordination to diverse member populations.
• Perform comprehensive evaluations and preventive assessments.
• Collaborate with physicians and other care team members; order tests when needed.
• Document medical plans, diagnoses and follow-up plans.
• Provide in-home and community-based care as required.
