Job summary

Molina Healthcare seeks a qualified Family Nurse Practitioner to provide screening, preventive and primary care in home health, community and facility settings. You will deliver care across adult to geriatric populations, coordinate care and document in EMR.

The role requires current FNP license, AANP/ANCC certification, a DEA license, BLS, and strong communication skills. Willingness to travel regionally and work with diverse populations is essential.

Qualifications

• At least 1 year of NP experience.
• Active state FNP license.
• DEA license active and unrestricted.
• BLS certification valid.
• Valid driver's license and reliable transportation.
• EMR and documentation proficiency.

Responsibilities

• Provide medical care and care coordination for adult, women’s health, pediatric, and geriatric populations.
• Perform H&P exams and preventive assessments.
• Manage chronic and acute primary care needs and determine urgency.
• Document diagnoses and care plans; coordinate with specialists.
