Qualifications

Responsibilities

• Provides general medical care and care coordination to diverse patient populations.
• Performs comprehensive evaluations including history and physical exams.
• Orders and interprets diagnostic tests; develops care plans.
• Coordinates with specialists and provides post-discharge follow-up.
• Conducts in-home visits and community-based clinics; uses EMR for documentation.
• Manages time and prioritizes multiple patient needs; travels regularly within assigned region.
