Qualifications
- Active and unrestricted FNP license and national certification.
- Current state license to practice as Family Nurse Practitioner.
- DEA license with authority to prescribe.
- BLS certification.
- Experience in home health or community settings preferred.
- Ability to travel up to 120 miles daily.
Responsibilities
- Provide general medical care and care coordination for diverse patient populations.
- Perform comprehensive history and physical exams and preventive care.
- Order and interpret diagnostic tests, coordinate with specialists.
- Deliver in-home and community visits, including nursing facilities.
- Document medical care in EMR and ensure timely records.
- Travel extensively within assigned region and respond to urgent needs.
