Qualifications

At least 1 year of nurse practitioner experience.
Active national certification from AANP or ANCC.
Current state Family Nurse Practitioner license (FNP).
DEA license with authority to prescribe.
BLS certification active and valid.
Valid driver's license with reliable transportation.
Ability to work in diverse settings and with varied populations.
Strong communication and problem-solving skills.
Proficiency with EMR systems.

Responsibilities

Provide general medical care and care coordination to adult, women’s health, pediatric, and geriatric patients.
Perform comprehensive history and physical exams.
Order and interpret diagnostic tests; refer to specialists as needed.
Develop and implement medical plans of care.
Schedule patient visits and coordinate post-discharge care.
Conduct in-person and telehealth visits; travel as required.
Maintain EMR documentation and participate in regional meetings.
May prescribe medications and perform procedures.
Local/state travel required.
