Job summary
Molina Healthcare is seeking a qualified Family Nurse Practitioner to provide preventive primary care and care coordination in home health, community sites, and pop-up clinics. You will perform exams, order tests, and develop care plans in collaboration with physicians, while traveling regionally.

Qualifications
- Active and unrestricted Family Nurse Practitioner license.
- Board certification with AANP or ANCC.
- DEA license with authority to prescribe in state of practice.
- Current BLS certification.
- Experience as an NP in home health or community settings preferred.
- Ability to travel up to 120 miles daily and sometimes overnight.
Responsibilities
- Provide comprehensive medical care and care coordination for diverse patient populations.
- Perform history, physical exams and preventive assessments.
- Order and interpret diagnostic tests within scope of practice.
- Develop and implement individualized care plans.
- Collaborate with physicians and other clinicians; recognize when oversight is needed.
- Deliver in-home, community, SNF, or pop-up clinic visits including tele-visit options.
