Job summary

Molina Healthcare Careers is seeking a Family Nurse Practitioner to provide screening, preventive primary care and care coordination for members in home health, community settings and pop-up clinics.

You will perform exams, manage chronic conditions, order tests, collaborate with physicians, and deliver post-discharge coordination. Travel up to 120 miles daily within the region may be required; an active FNP license, DEA, BLS and national certification are required.

Qualifications

• At least 1 year of experience as a nurse practitioner.
• Active and unrestricted national certification (AANP or ANCC).
• Current state-issued Family Nurse Practitioner license (active).
• DEA license with authority to prescribe (active).
• Current BLS certification.
• Valid driver’s license and reliable transportation for job-related travel.
• Ability to work in diverse settings with diverse populations.
• Ability to work independently with minimal supervision.
• Responsive in all forms of communication.
• Ability to remain calm in high-pressure situations.

Responsibilities

• Provide general medical care and care coordination for adult, women’s health, pediatric, and geriatric populations.
• Perform history and physical examinations and preventive assessments.
• Order/perform diagnostic tests and coordinate with physicians as needed.
• Develop and implement a medical plan of care and schedule visits.
• Provide post-discharge coordination to reduce readmissions.
• Travel up to 120 miles daily to sites; overnight stays possible.
• Document medical records in EMR.
• Collaborate with other NPs to develop best practices.
