Job summary

Molina Healthcare is seeking a Nurse Practitioner to deliver screening, preventive primary care and medical services to members in home health, community and nursing facility settings. You will coordinate care, perform evaluations, and implement medical plans across diverse populations.

Applicants must hold an active FNP license with AANP/ANCC certification, a DEA license, BLS, and be able to travel regionally. Strong communication and coordination skills are essential for success.

Qualifications

• Active and unrestricted national certification: AANP or ANCC.
• Current state license to practice as a Family Nurse Practitioner (FNP).
• DEA license with authority to prescribe.
• BLS certification.
• Driver's license and reliable transportation for travel requirements.
• Ability to work in diverse settings and with varied populations.

Responsibilities

• Provide general medical care and care coordination to targeted member populations.
• Perform comprehensive evaluations including history and physical exams.
• Address chronic and acute primary care concerns and determine urgency.
• Develop and document medical diagnoses and plans of care.
• Coordinate with specialists and order essential diagnostics.
• Conduct in-home and facility-based visits and post-discharge follow-up.
