Responsibilities

• Coordinate care among medical providers, employers, insurance carriers and injured workers.
• Attend appointments with injured workers at work sites or clinics.
• Develop, document, and monitor individualized recovery goals and return‑to‑work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers and internal teams.
