Job summary

ForzaCare, a nurse-led organization focused on helping injured workers recover and return to work, is seeking a Field Medical Case Manager in Illinois. You will coordinate care among injured workers, medical providers, employers, and insurers to ensure timely, transparent, and effective case management.

This role is ideal for licensed nurses or CRCs; travel to appointments is required. You will document recovery goals, monitor progress, attend visits, and ensure compliance with all regulatory

Qualifications

• Active RN or CRC license with college degree.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits at workplace or provider sites.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
