Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting them with medical providers, employers, and insurance carriers to ensure timely and transparent care management.

The role is ideal for licensed nurses or Certified Rehabilitation Counselors, with prior workers' compensation experience highly valued. Willingness to travel and meet clients on-site is required.

Qualifications

• Active RN or Certified Rehabilitation Counselor license required.
• CCM/CIRS and other case management credentials preferred.
• Must meet state licensure and certification requirements.
• Prior workers' compensation case management experience preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
