Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate recovery and a timely return-to-work for injured workers. You’ll link workers, medical providers, employers, and insurers to ensure effective care management.

Ideal candidates are licensed nurses or CRCs, with experience in workers’ compensation highly valued. Travel to appointments and compliance-driven work are required.

Qualifications

• Active RN or CRC license with associated degree is required.
• CCM or CIRS credentials preferred.
• Must meet state licensure requirements and be able to travel.

Responsibilities

• Coordinate care among medical providers, employers, insurers, and injured workers.
• Attend appointments at worksites or employer locations as needed.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance standards.
• Build relationships with clients, providers, and internal teams.
