Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate care among medical providers, employers, and insurers, serving as the central point of contact to ensure timely, transparent, and effective care management.

Ideal candidates are licensed nurses or Certified Rehabilitation Counselors (CRC). Prior workers' compensation experience is preferred. A valid driver’s license and ability to travel to appointments are required.

Qualifications

• Active RN or CRC license required.
• Bachelor's degree or higher.
• CCM/CIRS or similar credentials preferred.
• Experience in workers' compensation case management preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers and visit workplaces as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
