Qualifications

• Active RN or CRC license with associated college degree is required.
• Travel to appointments with clients using reliable transportation.
• CCM or CIRS or other case management credentials are preferred.
• Prior experience in workers' compensation case management is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at work sites or patients' locations as needed.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
