Job summary

ForzaCare is seeking a Field Medical Case Manager in Champaign, IL to guide injured workers through recovery and return-to-work processes. You will serve as the central point coordinating with medical providers, employers, and insurance carriers to ensure timely, transparent, and effective care management.

Ideal candidates are licensed RNs or CRCs with a background in workers' compensation; CCM/CIRS credentials are a plus.

Qualifications

• Active RN license or Certified Rehabilitation Counselor (CRC) with a college degree is required.
• CCM, CIRS, or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.
• Valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including at employers' worksites.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
