Ideal candidates hold an active RN or CRC license, with CCM/CIRS credentials preferred. Travel to appointments is expected as part of the role.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers at workplaces or clinics.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
