Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate care among medical providers, employers, and insurers, serving as the central point of contact.

This role is ideal for licensed nurses or certified rehabilitation counselors. Travel to appointments and a valid driver's license are required. Prior workers' compensation experience is preferred but not mandatory.

Qualifications

• Active RN or CRC license is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure requirements.
• Experience in workers' compensation case management is preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurers and injured workers.
• Attend appointments with injured workers at the workplace or other locations.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance standards.
• Build relationships with clients, providers and internal teams.
