Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will act as a central point of coordination among workers, medical providers, employers, and insurers to deliver timely, transparent case management.

Licensed nurses or CRCs are ideal fits, with preferred experience in workers' compensation. Willingness to travel to appointments is required as part of the role.

Qualifications

• Active RN or CRC license required.
• CCM, CIRS, or related case-management credentials preferred.
• Must meet state licensure requirements and travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
