Job summary

ForzaCare in the United States is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting them with providers, employers, and insurers to support timely recovery and return-to-work.

Ideal candidates are licensed as an RN or Certified Rehabilitation Counselor, with experience in workers' compensation preferred. You will travel to appointments as needed, develop individualized recovery goals, monitor progress, and keep stakeholders informed with clear documentation.

Qualifications

• RN or CRC license required; state licensure applicable.
• CCM, CIRS or related case-management credentials preferred.
• Prior workers' compensation case management experience is strongly preferred.
• Must be willing to travel to appointments.
• Ability to coordinate care among providers, employers, and insurers.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with the injured workers, which may include visiting employers and workers at the work site.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
