Job summary

ForzaCare, a nurse-led team in Winston-Salem, NC, seeks a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate care among medical providers, employers, and insurers to ensure timely, transparent case management.

The role is ideal for licensed nurses or certified rehabilitation counselors. Travel to appointments is required, and workers' compensation experience is strongly preferred.

Qualifications

• Active RN or CRC license is required.
• CCM/CIRS or other case management credentials are preferred.
• Travel to appointments and state licensure requirements must be met.
• Workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with the injured workers, including visits to employers or at the workplace.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
