Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will serve as the central point of coordination, connecting the injured worker, medical providers, employers, and insurers to ensure timely, transparent care management.

This role suits licensed RNs or CRCs. While prior workers' compensation experience is preferred, those with related backgrounds will be valued.

Qualifications

• Active RN or CRC license with a degree is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure requirements and travel.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers at their place of employment when needed.
• Develop, document, and monitor recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
