Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate care among medical providers, employers, and insurers to ensure timely, transparent, and effective case management.

This role is ideal for licensed nurses or certified rehabilitation counselors. While prior workers' compensation experience is strongly preferred, those who have it will find their background especially valuable in this role.

Qualifications

• Active RN or CRC license with associated college degree.
• CCM, CIRS, or other case management credentials preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including at employer worksites.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
