Qualifications

• BA/BS degree and a minimum of 2 years of experience working with a social work agency or equivalent.
• Experience with waiver programs (e.g., Family Care) preferred.
• Strong communication, documentation, and coordination skills required.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing.
• Identify members with potential clinical health care needs and coordinate with clinical teams as the single point of contact.
• Manage non-clinical needs to ensure cost-effective utilization of LTSS.
• Document member goals and care plans in collaboration with their care team.
• Submit utilization/authorization requests and report critical incidents as needed.
