Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician (RN Case Manager) for field-based work across Wisconsin. You will perform functional assessments, coordinate complex care, and develop comprehensive care plans to support members with chronic conditions and disabilities.

You will collaborate with health teams, coordinate authorizations, and ensure access to services while maintaining person-centered care.

Qualifications

• Requires an Associate of Science in Nursing or Bachelors of Science in Nursing and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience, which would provide an equivalent background.
• Current, active valid and unrestricted RN license in Wisconsin is required
• Minimum of one year experience working with at least one of the Family Care target populations is required.

Responsibilities

• Perform telephonic and face-to-face functional assessments for identification, evaluation, coordination and management of member's needs across health and social services.
• Manage members with chronic illnesses to ensure cost effective and efficient utilization of benefits.
• Obtain thorough member history to develop individualized care plans.
• Establish short and long-term goals with member and support network to address care needs.
• Develop and ensure access to services by creating person-centered care plans.
• Assist with authorizations/referrals within benefits or via appropriate arrangements.
• Interface with Medical Directors and care teams to optimize plan adherence.
