Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to operate in the field across Elkhart, Kosciusko and Marshall counties in Indiana. You will work under an RN supervision to assess, identify needs, and coordinate care across physical health, behavioral health, and long-term services.

Your role emphasizes direct engagement, care planning, and collaboration with the health care team to ensure efficient use of benefits and high-quality member outcomes.

Qualifications

• Minimum of 2 years of experience with chronic illnesses in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s).
• May require state-specific certification based on state law and/or contract.

Responsibilities

• Assist the responsible RN in identifying members for high risk complications.
• Provides all information collected to the RN, who verifies and interprets the information, conducts additional assessments, as necessary.
• Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by RN.
• Develop, monitor, evaluate, and revise the member's care plan to meet the member's needs.
