Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician for St. Joseph County, IN. This field-based role travels to client sites and occasionally attends training, working under an RN to coordinate long-term services for members with chronic conditions.

You must hold an LPN/LVN or social work license per state law, with a Nursing Diploma or AS in Nursing. At least 2 years in related care coordination is preferred, with MSW as a plus.

Qualifications

• Minimum requirements include a License (LPN/LVN, LSW, LCSW, LMSW or equivalent) per state law and a Nursing Diploma or AS in Nursing or related field.
• Minimum of 2 years of experience with individuals with chronic illnesses, co-morbidities, or disabilities in a Service Coordinator or similar role; or equivalent education/experience.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.

Responsibilities

• Assists responsible RN in identifying members for high risk complications.
• Obtains clinical data as directed by the responsible RN.
• Assists the RN in identifying members that would benefit from an alternative level of care or other waiver programs.
• Provides all information collected to the RN, who verifies and interprets the information, conducts additional assessments, and develops, monitors, evaluates, and revises the member's care plan.
• Participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by the RN, to ensure cost effective and efficient utilization of health benefits.
• Decision making based on current member needs, understanding disease processes, and application of clinical guidelines; nursing judgment not required.
