Job summary

Elevance Health seeks an LTSS Service Coordinator - RN Clinician to manage members’ care across the continuum in IN counties (Wayne, Union, Franklin). The role travels to client sites with hybrid work options and occasional office meetings, coordinating clinical assessments and care plans under licensure.

Qualifications

• HS diploma or equivalent.
• 3+ years in service coordination, case management, or similar roles with chronic illnesses, co-morbidities, and/or disabilities.
• Current, active RN license in Indiana.
• State-specific certification may be required by state law and/or contract.
• Associates/MA/MS in Health/Nursing preferred.
• Travel to worksite and other locations as necessary.

Responsibilities

• Identifies members for high risk complications and coordinates care with the member and health care team.
• Manages members with chronic illnesses to ensure cost-effective, efficient health benefits utilization.
• Obtains thorough member history to develop individual care plans.
• Sets short and long-term goals with the member, caregivers, family, and physicians.
• Identifies members that would benefit from alternative levels of care or other waiver programs.
• RN has overall responsibility to develop care plans and ensure access to services.
• Assist with implementation of care plans by facilitating authorizations/referrals within the benefits structure.
• Collaborate with Medical Directors and Inter-Disciplinary Teams on care plans.
• Assist in problem solving with providers, claims or service issues.
• Provide direction to LPN/LVN, social workers, and other licensed professionals in coordinating services.

Education

HS diploma or equivalent

RN license in applicable state

Associates/MA/MS in Health/Nursing preferred
