Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician in Lake County, IN. This field-based role travels to client sites and occasionally to offices, working under the supervision of an RN to coordinate long-term services and supports for members with chronic illnesses.

Responsibilities include data collection, assisting with care plans, and coordinating with the healthcare team to ensure appropriate and cost-effective care.

Qualifications

• LPN/LVN or equivalent license in applicable state.
• Minimum 2 years of experience coordinating care for individuals with chronic illnesses.
• Current, unrestricted LPN/LVN or social work license per state law.
• May require state-specific certification depending on law/contract.

Responsibilities

• Assist RN in identifying high-risk members.
• Obtain clinical data as directed by the RN.
• Help identify members needing alternative levels of care or waiver programs.
• Provide information to the RN for care planning and monitoring.
• Coordinate care for members with chronic conditions with the health team.
