Responsibilities

• Assist the RN with telephonic or face-to-face assessments for care coordination.
• Identify members for high-risk situations and contribute to care plans.
• Coordinate care for members with chronic illnesses and disabilities with the health care team.
• Support cost-effective and efficient utilization of health benefits.
